Challenges in Lyme Disease Diagnosis



A recent study published in the Journal of Clinical Microbiology: "Evaluation of standard and modified two-tiered testing algorithms using well-characterized early Lyme disease samples", sheds new light on a long-standing challenge in infectious disease diagnostics: early Lyme disease is still frequently missed by current laboratory testing.

The study assessed the performance of standard two-tiered testing (STTT) and modified two-tiered testing (MTTT) using well-characterized early Lyme disease patient samples. While both methods demonstrated excellent specificity (98–100%), their ability to detect infection in the earliest stage was markedly limited. Sensitivity at initial presentation ranged from only ~22% to 36%, meaning many true infections were not identified at the time of first testing. Although MTTT showed improved performance compared to STTT, neither approach was reliable enough to confidently rule out early disease.

Importantly, the findings highlight a critical biological limitation rather than a technical flaw: early infection often occurs before a detectable antibody response has developed. As a result, diagnostic performance improves over time, creating a vulnerable early window where serology may appear negative despite active infection.
